To receive a distribution from the assets of a company in liquidation,
a policyholder or other creditor must file a Proof of Claim with the
Liquidator's office. If you would like to request a Proof of Claim
form for a particular liquidation, you may do so online. Listed
below are all of the companies for which liquidation cases remain
currently open. Depending on the status of the liquidation, the
Liquidator may or may not be able to accept a Proof of Claim at this
